This set of sample forms includes a Job Action Sheet, which describes the duties of a Community Reception Center staff member assigned to work in the First Aid station. The staff assigned to the First Aid station will be required to document patients’ time in, time out, chief complaint, and general disposition. It also includes a sample First Aid log form.
